Complete the following interest form(s) prior to your purchase. This Form is your formal first registration step, along with your packet purchase. Once both are complete, you will receive a follow-up email from Cognito requesting further information ahead of your audition.
NOTE THAT THE SECOND COGNITO FORM WON'T BE SENT UNTIL THE PACKET IS DELIVERED. PACKETS ARE BEING DEVELOPED AND WILL BE DELIVERED AS SOON AS THEY ARE COMPLETE.
